After appling for an LLC, are there any monthly fees involved and how are they calculated ?

Typically there are no monthly fees (state or otherwise) required to maintain an LLC after formation. Depending on a state or formation you might be required to file an annual or biennial report, a franchise tax or any other annual filing to keep your LLC in good standing. In some states you also might need to file taxes (consult your accountant about that). And of course there is the annual fee for registered agent if you chose to use a commercial service for that, and federal tax returns.

(b) Your card balance is too low;
(c) Issuing bank has declined this transaction for some other reason related to your account.
